Providing supportive in-home services designed to provide personal assistance, training, and supervision for individuals with developmental disabilities who live with family, in their own home.

Core Service Components Include:

1. Personal Assistance Services (as defined in Virginia’s DD Waiver Manual)

  • Assistance with Activities of Daily Living (ADLs):
    • Bathing
    • Dressing
    • Grooming
    • Eating and feeding
    • Toileting
    • Mobility and transfers
  • Medication administration (as allowed under DMAS regulations and within provider scope)
  • Monitoring of health status and basic health-related tasks (e.g., vital signs, reminders)
  • Meal preparation and feeding assistance tailored to dietary needs
  • Household tasks essential to health and safety, such as laundry, cleaning, and organization

2. Skill Building and Training (aligned with Individual Support Plan – ISP)

  • Teaching and enhancing self-help skills (e.g., toileting, dressing independently)
  • Money management training (budgeting, making purchases)
  • Support with time management, daily routines, and task organization
  • Travel and mobility training (safe community navigation, public transportation use)
  • Personal safety training (e.g., responding to emergencies, fire safety)

3. Behavioral Support Implementation

  • Execution of Positive Behavior Support Plans (developed by licensed professionals)
  • Monitoring and reporting behavior data to behavior specialists or case managers
  • Crisis de-escalation techniques (when staff are appropriately trained)
  • Reinforcement of appropriate social and behavioral interactions

4. Socialization and Community Engagement (as authorized)

  • Development of communication skills, including support with augmentative communication devices (AAC)
  • Community integration support to participate in:
    • Recreational activities
    • Religious and cultural events
    • Volunteer and civic engagements
  • Facilitation of relationships and peer interactions in natural settings
  • Support in educational or employment-related activities (if part of the ISP)

5. Health Maintenance Activities (as per DMAS Guidance)

  • Support with healthcare follow-up, such as scheduling appointments
  • Accompanying individuals to medical or therapy appointments (when included in ISP)
  • Assistance with use and maintenance of durable medical equipment (DME) and assistive technology

6. Family/Caregiver Training and Support

  • Training for families/caregivers on implementing strategies outlined in ISP
  • Providing respite care (when included and authorized separately in ISP)
  • Regular updates and communication with families/caregivers on progress and needs
  • Support for caregiver self-care and planning

Service Coordination Activities

  • Collaboration with Support Coordinators (Case Managers) and other team members
  • Active participation in ISP meetings, assessments, and plan revisions
  • Detailed and timely documentation of:
    • Daily progress notes
    • Data collection on goals/objectives
    • Incident reports
    • Service logs for billing (per DMAS regulations)
